Just a few notes about this web page form:

Please fill out all the required fields and click the Submit button at the end of the form.
Supply only one address to be published to the on-line UConn Phone Book (either your personal name or registered e-mail address).

All Students using this form to change their personal name alias (ie. jonathan.doe@uconn.edu and jonathan.doe@huskymail.uconn.edu) must complete the following steps:

Fill out the Form below and Submit.
Wait for the change to appear in the UConn Phone Book.
Log in to the NetID website and reforward your mail (press the forward button at the bottom of the page).

Items in Bold are required! Examples are provided to right.

E-Mail Address (Required)

You must fill in your e-mail address and select No address change in order to register a Personal Name or Web Page.

Choose one: New Information Revised Information Delete Information

No address change (Data Required for Personal Name and Web Page Submissions).

Please Note: If you delete an e-mail address, your personal name will be deleted also!

	You may forward your mail to another server or Internet Service Provider if desired.)

Request a Personal Name

(If your e-mail address is already registered, you must still fill in your e-mail address and select No address change in order to request a personal name.) Before filling out this information, check the on-line directory to see what your current personal name is.

Choose one: New Information Revised Information Delete Information

	Please enter up to 5 possible personal name selections. The first available one will be assigned.	Examples:

Desired Personal Name:		quentin.doe
(Do not enter @uconn.edu)		j.doe
		john.doe
		john.q.doe
		j.quentin.doe

Register a Web Page

Choose one: New Information Revised Information Delete Information
